Recognising the Concern

Do You Notice These Changes?

Close-up of a person’s lower face and neck showing sagging, wrinkled skin with a neutral expression on the lips.

Loss of Firmness & Elasticity

Skin begins to feel looser and less toned, especially around the cheeks, jawline, and neck. Collagen and elastin decline with age, leading to visible sagging and a tired appearance.

Drooping Contours & Jowls

Facial definition softens as gravity pulls the skin downward. Jowls and folds may form along the jawline, and the lower face can appear heavier or less sculpted over time.

Understanding the Concern

What Causes Sagging Skin?

Understanding the causes of sagging skin is essential for effective prevention and treatment. Key factors include:

  • Ageing – Natural reduction in collagen and elastin production leads to decreased skin firmness.
  • Sun Exposure – U ltraviolet (UV) radiation accelerates the breakdown of collagen and elastin fibres, contributing to premature skin ageing.
  • Significant Weight Loss – Rapid or substantial weight loss can result in excess skin that lacks elasticity.
  • Smoking – Tobacco use impairs blood flow to the skin, depleting essential nutrients and leading to premature ageing and sagging.
  • Genetics– Inherited traits can influence skin elasticity and the rate at which sagging occurs.
  • Environmental Factors– Exposure to pollutants and toxins can damage skin structure over time.
  • Poor nutrition – A diet lacking in essential nutrients can weaken skin’s structural integrity.

Recognising these factors can guide preventive measures and inform treatment options to maintain firmer, more youthful skin.

THE DIFFERENCE

Is It Sagging Skin or Volume Loss?

Sagging Skin and facial volume loss often look similar both can create jowls, deeper folds and a tired appearance but they are different problems with different solutions.

  • Sagging is a loss of skin firmness and elasticity: the skin itself has loosened.
  • Volume loss is a change in the deeper structures of the face, where fat pads and bone support diminish with age, allowing the overlying skin to drape downward even when its elasticity is reasonable.

Many people have a combination of both, and choosing a treatment based on the wrong cause can mean disappointing results. Skin-tightening treatments such as Ultherapy, HIFU and Exilis work on laxity; they cannot replace lost structural volume.

This is why a proper assessment matters. During your consultation, Dr Joanna Teh examines your skin quality, facial structure and the pattern of change to identify what is actually driving the concern and recommends the approach, or combination, most likely to help.

If your concern is more about thin, finely wrinkled skin texture, see our crepey skin page. If it is loss of cheek fullness or facial definition, see facial volume loss.

Treatment Options

Approaches We May Discuss

Exilis Skin Tightening

A non-invasive skin tightening treatment that uses radiofrequency and ultrasound energy to stimulate collagen, firm sagging skin, and smooth wrinkles. Ideal for areas like the face, neck, and jawline, it helps improve skin texture and elasticity without surgery or downtime.

Ultherapy

Ultherapy is a non-surgical skin tightening treatment that uses focused ultrasound energy to stimulate collagen production deep beneath the skin’s surface. Often referred to as a non-invasive facelift, Ultherapy lifts and firms areas like the face, neck, and décolletage—without needles, downtime, or surgery.

Frequently Asked Questions

  • Sagging develops when the skin’s structural proteins — collagen and elastin — decline faster than the body replaces them.
  • Natural ageing is the biggest driver, but UV exposure accelerates the breakdown significantly, which matters in Perth’s high-UV climate. Other contributors include significant weight loss (the skin has been stretched and lacks the elasticity to retract), smoking, genetics and general skin health.
  • Gravity then acts on the weakened skin, producing jowls, looseness along the jawline and neck, and deeper folds around the mouth.
  • Mild to moderate skin laxity often responds well to non-surgical treatments that stimulate the skin’s own collagen production.
  • Technologies such as focused ultrasound (Ultherapy, HIFU) and radiofrequency (Exilis) deliver controlled energy into the deeper layers of the skin, triggering a repair response that gradually firms and lifts over the following months.
  • Results vary between individuals and depend on the degree of laxity, skin quality and age.
  • Significant excess skin for example after major weight loss may sit beyond what non-surgical treatment can achieve, which is something Dr Joanna Teh will assess honestly at consultation.
  • All three stimulate collagen, but they differ in energy type and depth. Ultherapy and HIFU use focused ultrasound that reaches the deeper foundational layer of the skin the same layer addressed in surgical lifting making them suited to lifting brows, jowls and the neck.
  • Exilis uses radiofrequency combined with ultrasound at shallower depths, which makes it well suited to tightening and smoothing skin texture, including delicate areas.
  • They are not interchangeable: the best choice depends on where your laxity sits and how your skin responds, which is exactly what a consultation determines.
  •
  • This is one of the most important questions to answer before treating anything.
  • Sagging is loosened skin; volume loss is diminished support beneath the skin — and they can look very similar, often appearing together as jowls or deepening folds.
  • Skin-tightening treatments address laxity but cannot replace lost volume, so treating the wrong cause wastes time and money.
  • At your consultation, Dr Joanna Teh assesses your skin quality and facial structure to identify what is actually driving the change and recommends the most appropriate pathway.
  • Because these treatments work by building your own collagen, results develop gradually over two to six months and can last one to two years or more, depending on the treatment, your age and how quickly your skin continues to age.
  • The new collagen is genuinely yours but the ageing process continues, so many patients choose periodic maintenance treatments to sustain their results.
  • Individual results vary, and realistic timeframes for your situation are discussed at consultation.
  • Most patients describe the treatments as tolerable. Ultrasound-based treatments (Ultherapy, HIFU) can produce brief moments of heat or tingling as energy is delivered, while Exilis is generally experienced as a warm sensation.
  • Comfort measures are available where appropriate. Most people return to normal activities the same day, though mild redness, tenderness or swelling can occur for a short period.
  • Your practitioner will explain what to expect for the specific treatment recommended to you.
  • There is no fixed age, it depends on your skin, genetics and sun exposure history.
  • Some people notice early laxity in their thirties; others not until their fifties.
  • Earlier intervention with collagen-stimulating treatments tends to work with more responsive skin, while more established sagging may need a staged or combined approach.
  • If sagging skin is starting to bother you, an assessment will tell you where your skin actually sits and whether treatment now, later or not at all is the sensible course.
  • It depends on the amount of excess skin. Skin that has been stretched by significant weight gain and loss often lacks the elastic capacity to retract fully, and non-surgical tightening works best on mild to moderate laxity rather than large folds of redundant skin.
  • Many post-weight-loss patients do benefit from collagen-stimulating treatments, particularly on the face and neck.
  • Dr Joanna Teh will give you an honest assessment of what non-surgical treatment can and cannot achieve in your case.
  • See also our post-weight loss rejuvenation page for a fuller discussion.

Learn more

Chin

Breakouts around the chin are often hormonally driven and can lead to lingering scars, texture irregularities, or pigmentation.

Learn more

A flat, receding or uneven chin can subtly impact your facial balance. Restoring shape and projection in the chin area can enhance harmony, confidence, and the way you’re perceived in profile.

Learn more

Horizontal lines or deep creases across the chin often develop with age or repeated muscle movement, contributing to a tired or aged look.

Learn more

Sun exposure or post-inflammatory changes can cause dark spots, uneven skin tone, or freckles to form on the chin area.

Learn more

Do you notice persistent redness or small red veins around your chin? These may be signs of broken capillaries or spider veins — a common concern that often becomes more visible with age or sensitivity. While subtle, they can affect how you feel in photos, under makeup, or in daylight.

Learn more

Jowls/Marionette Lines

Loss of skin firmness along the jawline and lower face can lead to the appearance of jowls and deepened marionette lines, often due to aging, collagen loss, or gravity.

Learn more

Marionette lines and jowls can create a downturned expression, making the face appear sad or fatigued even when well-rested.

Learn more

Chronic sun exposure accelerates collagen breakdown, leading to premature skin aging, uneven tone, and deeper lines around the mouth and jaw.

Learn more

Double Chin

A double chin refers to the appearance of excess fat or sagging skin beneath the chin and along the jawline. It can be caused by aging, genetics, poor skin elasticity, or weight gain. This concern may lead to a softer jawline and less facial definition, even in people with a healthy weight.

Learn more

Accumulation of fat under the chin that creates a “double chin” appearance.

Learn more

Loosened or lax skin under the chin and jawline, often due to ageing or weight changes.

Learn more

Smile lines, or nasolabial folds, are the natural creases that run from the nose to the mouth. They can become more prominent over time due to ageing and facial movement.

Learn more

Jaw Slimming

Reducing lower face fullness (often from muscle or fat) to create a more contoured, V-shaped appearance

Learn more

Laxity along the jawline can cause jowls and blur the definition of the lower face.

Learn more

Treatments aim to tighten the skin and enhance jawline definition for a sculpted, youthful look.

Learn more

Rejuvenation of the jawline with a focus on skin tightening and revitalising tired or heavy-looking lower face.

Learn more

Cheeks

Inflammatory acne and lingering scars on the cheeks can impact skin texture and confidence.

Learn more

Sun-induced spots or uneven skin tone often appear prominently on the cheeks, creating a mottled or patchy appearance

Learn more

A lack of definition or volume in the cheek area may affect overall facial balance and profile.

Learn more

Hormonal pigmentation commonly affects the cheeks in symmetrical brown or greyish patches, often triggered by sun and heat.

Learn more

Loss of volume and firmness in the cheeks can lead to a drawn or aged appearance.

Learn more

Redness, flushing, and visible veins frequently affect the cheeks, especially in sensitive or rosacea-prone skin.

Learn more

Lips

Fine lines, volume loss, and dryness around the lips can develop with age, affecting lip shape and definition

Learn more

Dark patches or uneven tone may appear on or around the lips due to sun exposure, hormonal changes, or inflammation

Learn more

Breakouts around the mouth and chin can lead to inflammation and scarring, often worsened by hormonal fluctuations or mask-wearing.

Learn more

Nose

Breakouts and post-inflammatory marks commonly appear on the nose due to excess oil and clogged pores

Learn more

Small brown spots or uneven skin tone caused by sun exposure can concentrate on the bridge and tip of the nose.

Learn more

Discolouration beneath the eyes that can make the area appear shadowed or tired.

Learn more

The nose often shows enlarged pores and blackheads due to high sebaceous activity and buildup.

Learn more

Though more common on the cheeks and forehead, melasma may appear on the nose as symmetrical brown patches triggered by hormones and sun exposure.

Learn more

Eyes

Fine lines that appear around the outer corners of the eyes, often more noticeable with facial movement or smiling.

Learn more

Discolouration beneath the eyes that can make the area appear shadowed or tired.

Learn more

A heaviness or sagging appearance of the upper eyelid area, which can contribute to a more tired or aged look.

Learn more

Mild swelling or puffiness under the eyes, often associated with fluid retention or skin changes over time.

Learn more

A common concern where the eye area may appear dull, puffy, or shadowed, often giving a fatigued or less refreshed appearance.

Learn more

Visible creases or folds beneath the eyes, which may become more noticeable with age or repeated facial expressions.

Learn more

Forehead

Improve skin texture and clarity by addressing active acne and reducing the appearance of post-acne marks.

Learn more

Learn More

Smooth and refresh the appearance of forehead lines caused by repetitive movement and ageing.

Learn more

Help reduce the appearance of deep creases between the brows caused by repeated frowning or stress.

Learn more

Fade the look of sun spots, freckles, and uneven pigmentation for a more even-toned complexion.

Learn more

Support the management of hormonally triggered pigmentation with tailored skin treatments.

Learn more

Enlarged pores on the forehead are often due to excess oil, aging, or sun damage, making the skin texture appear uneven.

Learn more

Calm visible redness and reduce flare-ups with treatments designed for sensitive, redness-prone skin.
